Before you leave, your doctor and the nursing team will inform you about further follow-up measures. You will either receive the discharge report upon discharge or it will be sent to your attending primary care physician.
Exit time is 10.00 a.m. Before you leave, your doctor will provide you with a prescription for your medications, a brief discharge report for your primary care physician, and additional therapy prescriptions and appointments as needed. To help you feel safe at home, the nurse will tell you what you should pay special attention to. If you transfer to post-hospital care facilities or outpatient care, you will also receive a nursing discharge report. We will assist you with ordering a cab or transporting your luggage if needed. You are welcome to obtain our prescribed medications from the pharmacy on the hospital grounds.
