Transcatheter therapy of degenerated surgical biological aortic and mitral valves (valve-in-valve TAVI)

Patients who have had a surgical biological heart valve replacement in the area of the aortic or mitral valve can also be treated with a transcatheter heart valve (valve-in-valve TAVI) if the heart valve has narrowed over the years or has become leaky (degenerated).

  • Valve-in-Valve TAVI: Implantation einer biologischen Katheterklappe in einer verengten (degenerierten) chirurgischen Aortenklappe mit einem Ballonkatheter-System..
  • Nach Valve-in-Valve TAVI: Biologische Katheterklappe in Aortenposition mit normalem Blutfluss von der linken Herzkammer zur Hauptschlagader Aorta.
  • Verengte (degenerierte) chirurgische Aortenklappe: Der Blutfluss von der linken Herzkammer in Richtung Hauptschlagader Aorta ist deutlich behindert.

In the area of the aortic valve, the procedure is largely identical to a normal transcatheter aortic valve implantation (see TAVI). If there is a risk of occlusion of a coronary artery, the procedure can also be performed using the BASILICA technique.

The access route differs in the area of the mitral valve. The procedure is performed via the inguinal vein and the atrial septum.

  • Valve-in-Valve TMVI: Implantation einer biologischen Katheterklappe in einer verengten (degenerierten) chirurgischen Mitralklappe mit einem Ballonkatheter-System über einen venösen Zugangsweg mit Passage des Vorhofseptums.
  • Nach Valve-in-Valve TMVI: Biologische Katheterklappe in Mitralposition mit normalem Blutfluss vom linken Vorhof in die linke Herzkammer.
  • Verengte (degenerierte) chirurgische Mitralklappe: Der Blutfluss vom linken Vorhof in die linke Herzkammer ist deutlich behindert.

If anatomically suitable, patients whose mitral valve has been surgically repaired and a mitral ring implanted (mitral valve reconstruction) can also be treated. The procedure is also performed via the inguinal vein and the atrial septum (valve-in-ring TAVI). A LAMPOON technique may be necessary to prepare for the procedure. This technique is offered at the USZ and only in a few other clinics worldwide.
